以短暂性脑缺血发作为初始表现的Stanford A型急性主动脉夹层患者报道1例

本病例报道1例以短暂性脑缺血发作为初发表现的Stanford A型急性主动脉夹层患者的临床诊疗情况。纳 入男性患者， 46岁，有高血压病史，表现为“突发左上肢麻木无力3小时”收入院。患者在入院前3小时突发左上肢麻 木无力，有头晕、胸闷、大汗淋漓，伴双眼视物模糊，无胸痛。入院神经系统查体未见异常。结合病史、体征及头颅 CT检查，初步诊断为短暂性脑缺血发作。入院再发左上肢麻木、伴轻微胸闷、头晕等，血压下降，完善全主动脉CTA 提示主动脉夹层（ Stanford A型），经抢救治疗无效而死亡。本文强调了以神经系统低灌注为主要表现的主动脉夹层的 临床特点，为非胸痛来诊的主动脉夹层类似病例的诊治提供临床资料，减少误诊、漏诊率。
