C-反应蛋白和降钙素原在儿童支气管肺炎中的联合诊断价值

目的： 探讨C-反应蛋白（ CRP）和降钙素原（ PCT）在儿童支气管肺炎中的联合诊断价值。 方法： 我院 收治的257例患支气管肺炎的儿童作为感染组，同时选取我院42名健康儿童作为同期健康对照组。检测出感染组和健 康对照组的CRP及PCT浓度值，用单因素方差（ one-way ANOVA）进行分析，比较感染组和健康对照组两组间CRP和 PCT值的变化；使用SPSS 17． 0软件进行绘制，分别得出CRP和PCT诊断儿童支气管肺炎的受试者工作曲线(ROC曲 线)，计算出CRP和PCT的AUC，即曲线下面积；然后分析ROC曲线，得出CRP， PCT单项诊断儿童支气管肺炎以及 CRP和PCT联合诊断儿童支气管肺炎的敏感度和特异性，判断单项检测和联合检测的诊断价值。 结果： 与健康组作比 较后发现， CRP和PCT在支气管肺炎患儿中均显著升高， CRP单独诊断儿童支气管肺炎的AUC为0.608，敏感度和特异 度为0.693 和0.405； PCT单独诊断儿童支气管肺炎的AUC为0.900，敏感度和特异度为0.996和0.143， CRP和PCT联合诊 断支气管肺炎的AUC为0.674，联合诊断的敏感度和特异度分别为0.996和0.095。 结论： CRP和PCT分别对儿童支气管 肺炎具有一定的诊断价值。二者联合对儿童支气管肺炎的诊断有一定的应用价值。
